30 Minutes - $45.00
60 Minutes - $75.00
75 Minutes - $95.00
90 Minutes - $110.00

On-site massage- $100.00 per hour with a two hour minimum.
(please add 7.275% state sales tax unless Dr's note, no tax on personal training)

We will give two weeks notice of any changes in services or fees. Payment of check or cash is due at time of service. Sorry we do not accept credit cards. We require 24 hours cancellation notice or full payment on all sessions. Your appointment time is reserved for you and cannot be refilled on short notice. Thanks for your consideration!

We will defer payment in cases of insurance reimbursement conditionally. Client must confirm that insurance will cover the sessions. We will provide receipts/charts, but client must bill insurance company directly. Client is responsible for full payment to practitioner. No discounts are allowed for insurance coverage, unless the sessions are prepaid. Client agrees to pay for any amount not covered by insurance.

(come at least once per month, Reg. $75, w/ tax $80.46).

May be shared with a family member, partner or spouse. Good for one practitioner only.
Sales tax may be exempt with doctor, psychologist or chiropractor's note for therapeutic purposes. All packages and gift certificates have expiration dates. If there is a request for refund on an unused package it will be prorated at the current full session price for used sessions, with any remaining unused amount being refunded. There are no refunds on gift certificates.
